To induce the Eastern Slope Aviation Academy (the “Academy”) to accept this application and subscription, the undersigned agrees and represents as follows:

All the information provided is true and accurate.

Represent and warrant that I have no known physical or psychological defects that would render me incapable to pilot an airplane (only applicable for flight instruction).

Hereby release and forever discharge the Eastern Slope Aviation Academy and all of its officers, directors, agents, and employees, acting officially or otherwise, from any and all claims, demands, actions or causes of action and loss, cost or damage arising from any injury to me or my property or my death that may result from any cause relating to the Academy's activities including without limitation the negligence of the Academy, or its officers, directors, employees or agents.

Accept financial responsibility for damage resulting to Academy equipment to the extent of the deductible portion of the insurance coverage or to the extend that my actions cause insurance coverage to be denied for any accident to an association-owned aircraft in which I am a student pilot or pilot-in-command or am otherwise legally responsible for such damage. It is recommended that those using Academy aircraft purchase an aircraft renters insurance policy.

Agree to abide by the applicable Federal Aviation Regulations and by the By- Laws of and Regulations of the Academy.

By participating in or attending any activity in connection with the Academy, I consent to the use of any photographs or video taken of me for use on the Academy website, social media, or other marketing material.
